Job Summary

CN Guidance and Counseling Services is seeking a highly motivated and compassionate Peer Specialist to join our team. As a Peer Specialist, you will play a vital role in supporting individuals with mental health and substance use conditions to achieve their recovery goals.

Key Responsibilities
  • Provide outreach and engagement services to clients, facilitating their access to mental health and substance use treatment services.
  • Offer health coaching and recovery support, empowering clients to take control of their well-being and make informed decisions about their care.
  • Assist clients with transportation to appointments, providing travel training as needed, and accompanying them to medical and other appointments to ensure their engagement and follow-through.
  • Maintain accurate and timely records of client interactions, services provided, and outcomes achieved.
  • Collaborate with interdisciplinary care teams to ensure coordinated and goal-directed care, including care transitions.
  • Participate in team meetings to provide input and feedback on client status, update plans and goals, and review outcomes to inform program development.
  • Act as a resource and consultant to team members on psychosocial and substance abuse issues and resources.
  • Provide telephonic and face-to-face outreach, engagement, and service planning in the field.
  • Link clients to community services, including medical, behavioral, residential, entitlement, and other needed services, as per interdisciplinary care plans.
  • Monitor service delivery to ensure coordination and continuity, advocating with service providers and resources as needed.
  • Provide crisis intervention and follow-up services to clients.
  • Facilitate data sharing with clients to promote partnered decision-making and informed care.
  • Participate in quality improvement training and activities, identifying and implementing process improvements to enhance client outcomes.
  • Encourage a culture of inclusion and respect, promoting diversity, equity, and access to services.
  • Adhere to safety protocols and procedures to ensure a safe working environment for all employees.
Requirements
  • High School Diploma or GED
  • Relevant life experience, including personal experience living with a disability and/or social/emotional challenge
  • Current New York State Driver's License
  • Clean DMV Record
  • Proof of Motor Vehicle Insurance
