CN Guidance and Counseling Services is seeking a skilled Nurse Specialist to join our team. As a key member of our Forensic ACT Team, you will play a vital role in delivering high-quality services to individuals with severe mental illness.
Key Responsibilities- Client Services Coordination: Schedule outside clinic appointments, lab appointments, and ensure the completion of all medical assessments for each client.
- Clinical Collaboration: Work closely with our psychiatrist to coordinate care, including clinical and educational training, medication administration, and supervision of psychiatric and medical treatment.
- Medication Management: Monitor the pharmacological effects of prescribed psychotropic medications and notify the psychiatrist of their reported effectiveness.
- Patient Education: Coordinate patient education efforts, including the benefits, risks, and possible side effects of prescribed medication.
- Medication Regimen Monitoring: Ensure completeness and accuracy in the provision of care.
- Team Supervision: Directly supervise Licensed Practical Nurses (LPNs) as needed, including weekly client schedules, medication delivery, and other tasks.
- Pager Coverage: Be available on an as-needed basis, 24/7, including pager coverage.
- Client Visits: Make a minimum of four to six client visits daily, chosen at the beginning of each workday and identified on the client work board.
- Documentation: Enter documentation for each client visit in the IMA System within 24 hours.
- Quality Improvement: Complete QI Training and participate in QI refresher training as needed.
