Position Overview

The role of the Community Resource Evaluator involves initiating the assessment process to evaluate individual needs and provide information on available options.

Job Responsibilities:
  • Conduct in-person assessments within the community to ascertain the individual's level of care, involving both the consumer and their caregiver/contact.
  • Engage and collaborate with caregivers, healthcare providers, and other relevant systems to accurately identify and address the individual's needs.
  • Offer options and referrals to meet individual requirements, particularly for those not pursuing Medicaid Waiver.
  • Document findings in the designated software system in accordance with established policies.
  • Work collaboratively with internal staff and external systems to ensure comprehensive support.
  • Advocate for clients, especially in situations involving health and safety concerns or risks of institutionalization.
  • Availability to conduct assessments during standard business hours, with occasional early evening or weekend hours as necessary to serve community needs.
Qualifications:
  • Current licensure as a social worker or registered nurse in the State of Ohio.
  • A minimum of two years of experience in home care, geriatrics, or medical social work, or other relevant experience as approved by the Ohio Department of Aging.
  • Basic understanding of chronic illnesses, aging processes, and medical terminology.
  • Strong verbal and written communication skills, particularly in interviewing and assessment.
  • Familiarity with local resources available for older adults.
  • Ability to communicate effectively and maintain positive working relationships.
  • Intermediate proficiency in Microsoft Office applications.
  • Capability to perform job duties remotely and in the field.
  • Ability to document information accurately and within required timeframes.
  • Proficiency in using the Ohio Department of Aging and Medicaid documentation systems, with training provided.
  • Must be able to transport a laptop and related equipment, lifting a minimum of twenty (20) pounds.
  • Ability to navigate various home environments, including those that may not be easily accessible.
  • Possession of a vehicle for work-related travel and a valid driver's license.
