Job Summary

CommuniCare Health Services Corporate is seeking a highly skilled Patient Assessment Coordinator to join our team. As a Patient Assessment Coordinator, you will play a critical role in identifying patients for potential admission into our facilities and maintaining positive relationships with current referral sources.

Key Responsibilities
  • Perform comprehensive clinical assessments to identify patients for potential admission
  • Coordinate transmission of clinical and benefit information from referral sources to our admissions teams, administrations, and third-party payers
  • Provide excellent customer service and accurate, timely, and comprehensive clinical assessments
  • Proactively work with all departments in the facility to achieve objectives
Requirements
  • Current, valid, and active license to practice as a Registered Nurse or Licensed Practical Nurse in the state of Ohio, or Social Work Degree, or other medical credential
  • Current, valid, and active driver's license required
  • Three (3) years of treatment experience in a Long Term Care setting preferred
  • Previous clinical screening experience preferred
  • Recent healthcare experience in Long Term Care preferred
  • Knowledge of local medical community preferred
  • Ability to collect, organize, evaluate, and communicate professionally and assertively, both verbally and in writing, pertinent clinical information with medical and administrative staffs, patients, and families
  • Strong customer service skills and commitment to service excellence
  • Outgoing, energetic personality
  • Ability to project a professional image
  • Knowledge of regulatory standards and compliance requirements
  • Strong organizational, prioritizing, and analytical skills
  • Ability to make independent decisions when circumstances warrant
  • Working knowledge of computer and software applications used in job functions
