Patient Evaluation Coordinator - RN / LPN / Social Worker

Skilled Nursing and Rehabilitation Facilities

CommuniCare Family of Companies is seeking a Patient Evaluation Coordinator to identify individuals for potential admission into our skilled nursing facilities. This role is essential in fostering positive relationships with existing referral sources. The Patient Evaluation Coordinator will streamline the admissions process by coordinating the transfer of clinical and benefits information from referral sources to the admissions teams, administration, and third-party payers.

Preferred Qualifications: Candidates should possess an RN or LPN license, a degree in Social Work, or other relevant medical credentials.

Estimated Salary Range: $60k-70k annually

Key Responsibilities:
  • Conducting clinical assessments
  • Serving as a resource for Liaisons
The Patient Evaluation Coordinator achieves these goals by delivering exceptional customer service and providing thorough, timely, and accurate clinical assessments while collaborating proactively with all departments within the facility.

Qualified candidates will have an RN or LPN license in the state, a Social Work degree, or other medical credentials. The ideal candidate will have at least three years of treatment experience in a Long Term Care environment. Previous experience in clinical screening is preferred, along with recent healthcare experience in a Long Term Care setting. Familiarity with the local medical community and a dynamic, engaging personality are also advantageous.

Job Requirements:
  • Preferred: Active RN or LPN license in Ohio, OR Social Work Degree, OR other medical credential.
  • Valid driver's license required.
  • Three years of treatment experience in Long Term Care preferred.
  • Prior clinical screening experience preferred.
  • Recent healthcare experience in Long Term Care preferred.
  • Knowledge of the local medical community preferred.
  • Ability to collect, organize, evaluate, and communicate clinical information professionally and assertively, both verbally and in writing.
  • Strong customer service skills and commitment to service excellence.
  • Outgoing and energetic personality.
  • Ability to project a professional image.
  • Knowledge of regulatory standards and compliance requirements.
  • Strong organizational, prioritizing, and analytical skills.
  • Ability to make independent decisions when necessary.
  • Proficient in computer and software applications relevant to job functions.
