CommuniCare Health Services is actively seeking a dedicated Patient Assessment Coordinator to identify potential patients for admission into our facilities. This role is crucial in fostering strong relationships with referral sources.

The Patient Assessment Coordinator will play a vital role in facilitating admissions by ensuring the smooth transmission of clinical and benefits information from referral sources to the admissions teams, administrative staff, and third-party payers.

Qualifications:

The ideal candidate will possess an RN or LPN license, a degree in Social Work, or other relevant medical credentials.

Salary Range: $60,000 - $70,000 annually

Key Responsibilities:

  • Conducting thorough clinical assessments
  • Serving as a resource for Liaisons

The Patient Assessment Coordinator is committed to delivering exceptional customer service and conducting accurate, timely clinical assessments. This role requires proactive collaboration with all departments within the facility.

Preferred Qualifications:

  • Active RN or LPN license in the State of Ohio, or a Social Work degree, or other medical credentials
  • Valid driver's license
  • Three years of treatment experience in a Long Term Care environment is preferred
  • Previous experience in clinical screening is advantageous
  • Recent healthcare experience in Long Term Care is preferred
  • Familiarity with the local medical community is a plus

  • Ability to professionally and assertively communicate clinical information with medical and administrative staff, patients, and families
  • Strong customer service orientation and commitment to service excellence

  • Knowledge of regulatory standards and compliance requirements
