The role of a Medication Administration Specialist is pivotal in ensuring the safe and effective delivery of medications to residents, while actively participating in the assessment and implementation of Individualized Service Plans (ISP).
Key responsibilities encompass the administration of prescribed medications, confirming the accuracy of medications, and meticulously documenting administration details. This position also involves collaborating with Resident Assistants (RA), Certified Nursing Assistants (CNA), and Licensed Practical Nurses (LPN) to support daily care routines.
Moreover, the Medication Administration Specialist is tasked with obtaining and recording vital signs and weights, monitoring residents for any medication-related responses, contributing to the development of care standards, and providing guidance to residents on social or personal matters.
Timely communication with healthcare providers, families, and LPNs regarding medication orders is essential, as is addressing residents' needs with urgency.

PRIMARY DUTIES AND RESPONSIBILITIES

The following responsibilities are typical for this position. This list is not exhaustive and additional duties may be assigned.

  • Assist and supervise the administration of medications to residents, ensuring proper ingestion or application.
  • Verify the identity of residents receiving medications, confirm the accuracy of the medication being administered, and document the time of administration in the Medication Administration Record (MAR) or electronic system.
  • Support RAs, CNAs, and LPNs with residents' daily routines, personal care, and housekeeping tasks as needed.
  • Obtain and document residents' vital signs (temperature, blood pressure, pulse, respirations, and oxygen saturation) and weight as instructed.
  • Monitor residents for responses to medications and prepare reports or notify designated personnel of any unexpected reactions.
  • Contribute to the formulation and evaluation of care standards.
  • Provide counseling to residents in identifying and resolving social or personal issues.
  • Notify families when it is necessary to reorder mail-order medications.
  • Communicate with healthcare providers regarding medication orders and inform on-shift LPNs of any required changes.
  • Ensure that call lights are answered promptly to meet residents' needs as soon as possible.
  • Maintain accurate counts of narcotics and document in accordance with policy.
  • Assist with dietary responsibilities, including serving meals and cleaning dining areas as required.
  • Attend all mandatory training and meetings to remain informed of changes within the organization and nursing department.
  • Complete all state-required training and documentation annually.
  • Ensure all caregiver duties are performed as assigned and documented appropriately in the electronic record.
  • Participate in the care planning process as assigned.
  • Address concerns from employees, families, and residents to foster satisfaction and effective communication.
  • Be knowledgeable of the fire evacuation and disaster policies of the community and participate in drills as assigned.
  • Maintain confidentiality of all personal and health information regarding residents and staff.
  • Conduct oneself in a professional manner in accordance with facility policies and procedures.
  • Perform all other duties as assigned by the supervisor or Executive Director that contribute to the success of Remember Me Senior Care and the well-being of residents.

QUALIFICATIONS

Educational Background and Experience:

  • High school graduation or equivalent is mandatory.
  • Previous experience as a Certified Nursing Assistant or student nurse is preferred.
  • Experience working with residents with Alzheimer's or Dementia is advantageous.
  • Proficiency in reading, speaking, and writing in English is required.

Certifications, Licenses, and Special Requirements:

  • Completion of a certification program for Nursing Assistants is required.
  • Successful completion of Medication Aide Training is necessary.
  • Current certification in CPR and first aid is mandatory.
