Role Summary
The Patient Care Coordinator I plays a vital role as a professional intermediary for patients, families, healthcare providers, and various departments within Fred Hutch. This position requires critical thinking skills to manage and streamline complex scheduling requests utilizing multiple computer systems. The coordinator ensures continuity of care by facilitating smooth transitions between different care environments and building trusted relationships with patients to address their unique needs.


Key Responsibilities
  • Manage all patient appointment scheduling related to treatment at Fred Hutch, including external consultations and referrals, while directly communicating schedules to patients.
  • Collaborate with patients, families, visitors, providers, and staff to identify needs, resolve issues, and provide necessary information.
  • Update patient and caregiver contact details in the electronic health record (EHR) as required.
  • Communicate effectively with healthcare providers regarding patient scheduling needs and treatment plans.
  • Prioritize and manage scheduling requests efficiently.
  • Recognize emergency situations and initiate appropriate procedures.
  • Support departmental functions and may undertake additional responsibilities such as cross-coverage and special projects.
  • Assist in training new staff members.
  • Update provider templates for immediate scheduling needs.
  • Handle calls from referring physicians, patients, and family members seeking care at Fred Hutch.
  • Collect and input patient demographic and insurance information into registration systems.
  • Assist with financial clearance processes as necessary.
  • Coordinate access to the Commercial Cell Therapy Portal (IMTX).
  • Delegate tasks to the Program Assistant and/or Patient Care Coordinator Assistant as appropriate.

Qualifications

Required:

  • High school diploma or equivalent.
  • Ability to work independently while organizing and prioritizing multiple responsibilities.
  • Strong verbal and written communication skills.
  • Proficiency in word processing (MS Word), email, and data entry in a Windows environment.
  • Demonstrated customer service capabilities.
  • Problem-solving skills to meet patient needs effectively.
  • Ability to thrive in a fast-paced, complex healthcare setting.
  • Attention to detail to ensure accuracy in scheduling.
  • Active listening and relationship-building skills.
  • Strong coping skills and the ability to triage issues appropriately.

Preferred:

  • Bachelor's degree (BA or BS).
  • At least 2 years of experience in a patient care environment.
  • Familiarity with Epic or similar scheduling and registration software.
