Role Summary

The Patient Care Coordinator I plays a vital role within a collaborative team, serving as a professional liaison for patients, families, care providers, and various departments within Fred Hutch. This position requires strong critical thinking skills to manage and execute complex scheduling tasks using multiple computer systems. The coordinator ensures continuity of care by facilitating smooth transitions between different care environments and building trusted relationships with patients to address their unique needs.

Key Responsibilities

  • Manage all patient appointment scheduling related to treatment at Fred Hutch, including external consultations and referrals, while directly communicating schedules to patients.
  • Collaborate with patients, families, visitors, providers, and staff to identify needs, resolve issues, and provide necessary information.
  • Update patient and caregiver contact details in the electronic health record (EHR) as required.
  • Communicate effectively with healthcare providers regarding patient scheduling and treatment requirements.
  • Prioritize and manage scheduling requests efficiently.
  • Recognize emergency situations and initiate appropriate response protocols.
  • Support departmental functions and may undertake additional responsibilities as needed.
  • Assist in training new staff members.
  • Manage provider templates for immediate scheduling needs.
  • Handle inquiries from referring physicians, patients, and family members seeking care.
  • Collect and input patient demographic and insurance information into registration systems.
  • Facilitate the financial clearance process as necessary.
  • Coordinate access to the Commercial Cell Therapy Portal (IMTX).
  • Delegate tasks to the Program Assistant and/or Patient Care Coordinator Assistant as appropriate.

Qualifications

Required:

  • High school diploma or equivalent.
  • Ability to work independently while organizing and prioritizing multiple responsibilities.
  • Effective verbal and written communication skills.
  • Proficiency in word processing (MS Word), email, and data entry in a Windows environment.
  • Strong customer service abilities.
  • Problem-solving skills to meet patient needs.
  • Capability to function in a high-pressure, deadline-driven healthcare setting.
  • Attention to detail to ensure accurate scheduling.

Preferred:

  • Bachelor's degree (BA or BS).
  • At least 2 years of experience in a patient care environment.
  • Familiarity with Epic or similar scheduling and registration software.
