The Care Coordinator plays a vital role in supporting the population health team by managing incoming referrals and monitoring outreach efforts. This position involves making proactive calls and sending messages to patients through Mychart to arrange appointments, offer resources, and coordinate care. The Care Coordinator is also responsible for providing patients with preparation instructions for classes and visits.
In addition, this role includes handling incoming inquiries from patients, healthcare providers, and clinic staff. The Care Coordinator compiles and analyzes data to prepare and submit reports related to program performance and organizational metrics. Responsibilities also include scheduling venues for educational sessions and offering routine financial counseling while addressing referral and billing inquiries.
The Care Coordinator ensures the setup of necessary equipment and resources for classes and takes minutes during departmental meetings. This position also encompasses various clerical duties as assigned by the supervisor.
Qualifications Required:
- Driving may be necessary for this role; compliance with state driving laws is mandatory. Proof of driver’s license and auto insurance will be required upon request.
- A minimum of 2 years of experience in a healthcare setting, particularly in ambulatory practice.
- At least 2 years of driving experience.
- Associate's Degree.
- Experience with Excel spreadsheets.
- One year of experience with coding systems such as ICD-9, ICD-10, CPT, or HCPCS.
- Familiarity with electronic medical records (EMR).
- Experience in a liaison role among patients, providers, and staff.
