CenterWell Home Health is looking for a dedicated Social Worker/Case Manager, also known as a Community Resource Coordinator (CRC), to enhance the lives of members in the Family Care program. In this role, you will collaborate with colleagues to deliver exceptional care and support to our members, assisting them in navigating their daily challenges.

The CRC is responsible for providing comprehensive social service management for elderly individuals and adults with intellectual, developmental, or physical disabilities who qualify for the Family Care program.

  • Identify the member's priorities and assist them in achieving their goals.
  • Work closely with a Health and Wellness Coordinator (RN) to ensure ongoing collaboration regarding the member's care plan.
  • Conduct face-to-face social assessments for members upon enrollment and at least every six months thereafter in their preferred environment.
  • Coordinate and document quarterly in-person visits and monthly phone check-ins.
  • Engage in the development and continuous review of the member's care plan.
  • Lead initiatives related to social integration, employment, housing, and other non-health-related matters.
  • Promote wellness and prevention strategies while addressing identified risks.
  • Maintain accurate documentation and complete necessary paperwork.
  • Travel is a requirement of this position, with mileage reimbursement available.

Required Qualifications

  • A four-year degree in Human Services or a related field with a minimum of one year of experience working with at least one of the Family Care target populations OR a four-year degree in another field with three years of relevant experience in the same area.

Preferred Qualifications

  • Certification in social work.

Additional Information

  • Workstyle: This is a field-based position where employees perform their primary responsibilities at various locations, including member homes and partner facilities.
  • Travel: Up to 40% travel required within the designated service area.
  • Typical Work Days/Hours: Monday to Friday, 8:00 AM to 4:30 PM CST.

Driving Requirements:

This role is part of the driver safety program, necessitating a valid state driver's license and proof of personal vehicle insurance with specified coverage limits.
