CenterWell Home Health is looking for a dedicated Social Worker/Case Manager, also known as a Community Resource Coordinator (CRC), to support members in our Family Care program. In this position, you will collaborate with colleagues to deliver exceptional care to our members, assisting them in navigating their daily lives. The CRC is responsible for providing comprehensive social service management for elderly individuals and adults with intellectual, developmental, or physical disabilities who qualify for the Family Care program.

  • Identify what matters most to the member and assist them in achieving their goals.
  • Work alongside a Health and Wellness Coordinator (RN) to continuously collaborate on the member's care plan.
  • Conduct in-person social assessments for members at enrollment and every six months thereafter in their environment.
  • Organize, execute, and monitor quarterly in-person visits and monthly phone check-ins.
  • Engage in the creation and ongoing evaluation of the member's care plan.
  • Lead initiatives related to social integration, employment, housing, and other non-health-related matters.
  • Provide education on wellness and prevention strategies to reduce identified risks.
  • Maintain accurate documentation and complete necessary paperwork.
  • Travel is required, and mileage reimbursement is available.

Required Qualifications

  • A four-year bachelor's degree in Human Services or a related field with a minimum of one year of experience working with at least one of the Family Care target populations OR a four-year bachelor's degree in any other discipline with three years of relevant experience with at least one of the Family Care target populations.

***The Family Care target population includes: elderly individuals and adults with intellectual, developmental, or physical disabilities.***

  • Certification in social work.

Additional Information

  • Workstyle: This is a field-based role where employees perform their primary responsibilities at various locations, such as providing services at partner facilities or members' homes.
  • Work Location: Remote position with specific geographic requirements.
  • Travel: Up to 40% within the designated area.
  • Typical Work Days/Hours: Monday to Friday, 8:00 AM to 4:30 PM CST.

Internet Requirements:

To ensure effective remote work, employees must have internet service that meets the following criteria:

  • A minimum download speed of 25 Mbps and an upload speed of 10 Mbps is recommended; a wired cable or DSL connection is preferred.
  • Satellite, cellular, and microwave connections may only be used with prior approval.

Driving Requirements:

This role is part of our driver safety program, requiring a valid state driver's license and proof of personal vehicle liability insurance with specified limits.
