Community Resource Coordinator

CenterWell Home Health is looking for a Social Worker/Case Manager, also known as a Community Resource Coordinator (CRC), to support members in our care programs. In this position, you will collaborate with colleagues to deliver exceptional care to our members, assisting them in navigating their daily lives. The CRC is responsible for providing comprehensive social service management for elderly individuals and adults with intellectual, developmental, or physical disabilities.
- Identify what matters most to the member and assist them in achieving their goals.
- Work alongside a Health and Wellness Coordinator (RN) to ensure ongoing collaboration regarding the member's care plan.
- Conduct in-person social assessments for members upon enrollment and at least biannually thereafter in their own environments.
- Organize, execute, and monitor quarterly in-person visits and monthly phone check-ins.
- Engage in the development and continuous evaluation of the member's care plan.
- Lead initiatives related to social integration, employment, housing, and non-health-related concerns.
- Promote wellness and preventive measures while addressing identified risks.
- Maintain accurate documentation and complete necessary paperwork.
- Travel may be required, with eligibility for mileage reimbursement.
Utilize your expertise to create meaningful change
Required Qualifications
- A four-year bachelor's degree in Human Services or a related field with a minimum of one (1) year of experience working with at least one of the target populations OR a four-year bachelor's degree in any other discipline with three (3) years of relevant experience.
Preferred Qualifications
- Certification in social work.
Additional Information
- Workstyle: This position is primarily field-based, with responsibilities carried out at various locations, including member homes.
- Travel: Up to 40% travel may be required.
- Typical Work Days/Hours: Monday to Friday, 8:00 AM to 4:30 PM.
Driving Requirements
This role is part of our driver safety program, necessitating a valid state driver's license and proof of personal vehicle insurance.
