Columbia River Mental Health Services is seeking a skilled Nurse Care Coordinator to join our team. As a key member of our Medication-Assisted Treatment (MAT) program, you will play a vital role in providing comprehensive care to individuals struggling with Opioid Use Disorder (OUD).
Key Responsibilities- Provide first contact with potential MAT clients, including phone calls, program information, and scheduling appointments.
- Manage day-to-day care of clients, supporting their recovery from OUD and addressing complex biopsychosocial problems.
- Administer medication per agency/state/federal regulations and provide education on infectious diseases, including hepatitis C.
- Work collaboratively with counseling staff to encourage retention and success in treatment.
- Participate in staff education and training, as well as community partnerships and outreach.
- Valid Washington State license as a registered nurse or licensed practical nurse.
- Minimum of 2 years full-time experience in direct treatment of persons enrolled in a medication-assisted treatment program.
- Strong interpersonal and decision-making skills, with excellent written and oral communication skills.
