Care Management Coordinator

As a Care Management Coordinator with Area Agency on Aging & Disabilities of Southwest Washington, you will play a vital role in supporting adults with disabilities and older persons to maintain their independence in the community. Your primary responsibility will be to assess client needs, authorize and obtain in-home and community-based services to prevent institutionalization and promote timely returns home following hospital or residential stays.
Key Responsibilities
- Develop and implement individualized care plans to meet client needs and objectives.
- Coordinate services with other community service providers, including mental health and disability organizations.
- Monitor the TXIX CM phone line, responding to client concerns and triaging situations with assigned case managers.
- Collaborate with in-house nursing staff for case consultation, service plan development, and provider oversight.
- Provide education and support to clients, families, and caregivers on local in-home and community options and resources.
Requirements
- Bachelor's degree in behavioral or health sciences.
- Two years of paid on-the-job social service experience or equivalent combination of education and experience.
- Ability to communicate verbally in English and produce written documents with proper English sentence construction, punctuation, and grammar.
- Computer and software skills, with ability to operate general office equipment.
