MDS Coordinator

As a skilled MDS Coordinator at Therapy Solutions, you will play a vital role in enhancing the lives of our residents by providing high-quality care and support. Our warm and inviting 88-bed skilled nursing community is dedicated to delivering exceptional care and services to our residents and their families.
Key Responsibilities- Review patient care plans to ensure they are up-to-date and meet the needs of our residents.
- Collaborate with our interdisciplinary team to develop and implement individualized care plans.
- Monitor and guide the completion of PPS and OBRA assessments to ensure accurate and timely data.
- Manage the Utilization Review (UR) process to ensure efficient and effective care delivery.
- Oversee MAR's treatment, flow sheets, and physician orders to ensure seamless care coordination.
- Bachelor's degree in Nursing from an accredited college or university.
- Current, unencumbered, active license to practice as a RN or LPN in this state.
- MDS Certification preferred.
- Knowledgeable and experienced in the RAI process, CAAs, RUG categories, and MDS 3.0.
- Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities.
