Care Coordinator Job Description

Columbia River Mental Health Services is seeking a skilled Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in providing comprehensive care coordination services to individuals with chronic health conditions.

Key Responsibilities:
  • Initial contact and screening of referred individuals using appropriate tools and motivational interviewing techniques
  • Assistance to participants in navigating the healthcare system
  • Education on self-management health tools
  • Coaching for participant/family to ensure understanding of the Health Action Plan
  • Assistance to Participant/family in accessing community support groups and wellness education programs
  • Coordinating and mobilizing treating and/or authorizing entities
Community Outreach:

The Care Coordinator will maintain ongoing relationships with community agencies that refer to CRMHS for clinical services or provide ancillary services to CRMHS clients. This includes individual and team meeting attendance, as well as presentations to community groups, including contract and governmental organizations, non-profit, and healthcare clinics.

Requirements:
  • Approximately 1-2 years of experience in the social service field with mental illness or chemical dependency preferred
  • Good communication and interpersonal skills
  • Basic knowledge of chronic mental illness, customer service, and maintaining and building therapeutic relationships
  • Assessment skills, multi-tasking, making decisions, problem-solving, intermediate computer literacy, and good time management skills
Benefits:
  • Competitive salary range: $23.92-$26.57/hour
  • 11 paid holidays per year, 2 paid floating holidays, and 5 weeks of PTO days in the first year that grows year after year
