Interborough Developmental and Consultation Center is seeking a highly skilled Care Coordinator to join our team. As a Care Coordinator, you will play a critical role in ensuring that our clients receive the necessary medical, social, and psychosocial services to maintain their independence and quality of life.
Key Responsibilities:
- Conduct home visits, field visits, phone, and mail outreach to individuals referred for enrollment.
- Review client intake assessments and use results to coordinate the completion of care plans, self-management goals, and strategies; document them in the electronic medical record.
- Travel to and visit consumers' residences or apartments to provide counsel and assistance to help consumers maintain the greatest degree of independent living.
- Work closely with the interdisciplinary care team, including primary care physicians, psychiatrists, therapists, residential services, and substance abuse treatment programs.
- Provide input to providers, clients, and families for written individualized care plans.
- Attend care coordination meetings, collaborate with outreach staff and external care coordinators, assess consumer needs, and discuss/advocate for needed services.
Requirements:
- Bachelor's degree required; three years of experience or two years of experience as a Health Home care manager in the SMI or SED population.
- Credentialed Alcoholism and Substance Abuse Counselor (CASAC) with two years of experience.
- Strong written and verbal skills.
- Excellent computer skills, demonstrated ability to use word-processing and database programs.
- Knowledge of community resources and entitlements/benefits.
- Good organizational and interpersonal skills.
- Bilingual Spanish/Russian encouraged to apply.
