Job Summary

We are seeking a skilled Physical Therapist to join our team. As a Physical Therapist with Granite Peak Home Health, you will be responsible for providing personalized physical therapy services to patients in their homes. Your expertise will help patients recover from illness, injury, or surgery, and improve their overall quality of life.

Responsibilities

  1. Assess and Develop Patient Care Plans

Conduct thorough assessments of patients' physical abilities and develop personalized care plans to address their unique needs and goals.

Provide Physical Therapy Services

Deliver high-quality physical therapy services to patients in their homes, including exercises, stretches, and other interventions to improve mobility, strength, and function.

Collaborate with Healthcare Team

Work closely with other healthcare professionals, including physicians, nurses, and social workers, to ensure comprehensive patient care and coordination.

Document Patient Progress

Maintain accurate and detailed records of patient progress, including treatment plans, progress notes, and outcome measures.

Communicate with Patients and Families

Provide education and support to patients and their families on physical therapy techniques, home exercise programs, and other aspects of patient care.

Requirements

  • Graduate from a physical therapist curriculum approved by the Commission on Accreditation in Physical Therapy Education, American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council of Medical Education of the American Medical Association and American Physical Therapy Association.
  • Currently licensed in the state(s) in which practicing
  • Two (2) years' experience, preferred.
  • Acceptance of philosophy and goals of this Agency.
  • Ability to exercise initiative and independent judgment
