Job SummaryWe are seeking a skilled Registered Community Care Nurse Case Manager to join our team. In this role, you will work collaboratively with a multidisciplinary team to plan, coordinate, and supervise the delivery of services to individuals enrolled in the Home and Community Based Services Program.
Key Responsibilities- Develop and implement individualized care plans to meet the unique needs of program participants.
- Coordinate with healthcare providers, community resources, and family members to ensure seamless care delivery.
- Monitor and evaluate the effectiveness of care plans, making adjustments as needed.
- Communicate effectively with program participants, families, and healthcare teams to ensure informed decision-making.
- Manage and maintain accurate records, reports, and documentation.
- Stay up-to-date with relevant laws, regulations, and industry best practices.
- Graduate of an accredited nursing program, Bachelor of Science in Nursing (BSN) preferred.
- Current state licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) if state-approved.
- Certification in Case Management is highly desirable.
- Minimum three (3) years of professional nursing experience, with a focus on case management.
- Knowledge of case management principles, practices, and procedures.
- Understanding of diagnostic and crisis intervention skills.
- Knowledge of long-term care consumer needs and issues.
- Experience with budget management and Medicare/Medicaid reimbursement.
- Understanding of human behavior, disabilities, and the aging process.
