As a Registered Community Care Nurse Case Manager, you will be an integral part of our team, working collaboratively to provide comprehensive services to individuals enrolled in our Home and Community Based Services Program.
Key Responsibilities- Develop and implement individualized care plans, ensuring seamless coordination and delivery of services.
- Collaborate with multidisciplinary teams to address the unique needs of our patients, promoting holistic care and support.
- Manage and supervise the provision of services, ensuring accuracy, efficiency, and adherence to regulatory requirements.
- Communicate effectively with patients, families, and healthcare professionals, fostering strong relationships and trust.
- Stay up-to-date with industry developments, best practices, and regulatory changes, applying this knowledge to enhance patient care and outcomes.
- Graduate of an accredited nursing program, with a Bachelor's degree in Nursing preferred.
- Current state licensure as a Registered Nurse or LPN, with certification in Case Management a plus.
- Minimum of three years of professional nursing experience, with a strong background in case management and community care.
- Proven knowledge of case management methods, practice, and procedure, as well as diagnostic and crisis intervention skills.
- Experience with budget management, Medicare and Medicaid reimbursement, and human behavior, disabilities, and the aging process.
