Job Summary

We are seeking a skilled Physical Therapist to join our team. As a Physical Therapist with Granite Peak Home Health, you will be responsible for providing personalized physical therapy services to patients in their homes. Your expertise will help patients recover from illness, injury, or surgery, and improve their overall quality of life.

Key Responsibilities

  • Assess and Develop Patient Care Plans

Conduct thorough assessments of patients' physical abilities and develop personalized care plans to address their unique needs and goals.

Provide Physical Therapy Services

Deliver high-quality physical therapy services to patients in their homes, including exercises, stretches, and other interventions to improve mobility, strength, and function.

Collaborate with Healthcare Team

Work closely with other healthcare professionals, including physicians, nurses, and social workers, to ensure comprehensive patient care and coordination.

Document Patient Progress

Maintain accurate and detailed records of patient progress, including treatment plans, progress notes, and outcome measures.

Comply with Regulatory Requirements

Ensure compliance with all relevant laws, regulations, and standards governing home health care services, including OASIS assessments and ICD-10 coding.

Requirements

  • Graduate Degree in Physical Therapy

A graduate degree in physical therapy from an accredited program.

Current Licensure

Current licensure to practice physical therapy in the state(s) where practicing.

Experience

At least two years of experience as a physical therapist, preferably in a home health care setting.

Strong Communication Skills

Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
