About the Role

We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as an Admissions Coordinator in Hospice Care. As a key member of our interdisciplinary team, you will play a vital role in coordinating the admissions process for patients and their families.

Responsibilities
  • Coordinate admissions of hospice patients by conducting skilled assessments of their physical, psychosocial, and spiritual needs.
  • Collaborate with the attending physician, medical director, and interdisciplinary team to establish the initial plan of care.
  • Provide skilled interventions and instructions to patients and caregivers to ensure optimum symptom control and patient comfort under the direction of the physician.
  • Function as a liaison between disciplines and facilities during the admission process.
  • Develop, prepare, and maintain accurate and timely individualized patient care progress records.
  • Communicate significant findings, problems, changes in condition, or environment to the clinical manager/physician and other members of the hospice care team.
  • Perform on-call responsibilities and provide on-call services to patients/families as assigned.
Requirements
  • Current RN license, specific to the state(s) you are assigned to work.
  • One year of clinical RN experience.
  • Current CPR certification.
