Brookhaven Hospice is seeking a Full-Time Compassionate Admission Nurse (RN) to deliver exceptional patient care in various locations.

Work Schedule:
Tuesday - Saturday, 10am - 6pm


As an Admission Nurse (RN), you will play a crucial role in representing our Hospice programs to patients, families, and stakeholders through pre-assessments, information dissemination, and admissions either in a patient's residence or a healthcare facility.

This role involves collaboration with the intake team, referring physicians, and other sources to uphold the hospice philosophy of care.

Key Responsibilities:
- Review clinical referral information and respond to requests and inquiries as needed. Collaborate with the Clinical Director regarding the eligibility and suitability of clients for hospice services.


- Provide information, pre-assessment, and admission services to patients referred to hospice; conduct pre-assessment visits in homes or healthcare facilities as required.


- Communicate with the Intake Department, referring physicians, and the Hospice Medical Director to ensure the appropriateness of each referral and that necessary certifications are secured.

- Apply nursing principles and techniques in patient treatment. Deliver nursing care, emotional support, and education to patients and their families. Encourage patient and family involvement in the care plan to achieve established goals.

- Participate in On-Call duties as assigned by the Clinical Director and Program Director.

Qualifications:
- Bachelor's degree in nursing or healthcare preferred.

- Hold a current unencumbered license as a Registered Nurse.

- Minimum one (1) year of experience as an RN in a medical-surgical/acute care setting.

- Experience in Hospital and/or Hospice/Palliative/Home Health preferred but not mandatory.

- Proficient in computer skills, including typing and various software programs, with a background in EMR.

- Maintain current CPR certification.
