Key Responsibilities:- Perform initial assessments with patients and their families in diverse environments
- Communicate the hospice philosophy, available services, and Advance Directives
- Evaluate the needs of patients and families for hospice care
- Provide guidance and education to patients and their families
- Secure necessary consents and authorizations for care plans
- Facilitate smooth transitions from hospitals to hospice care
- Registered Nurse (RN) with a valid New York State License
- At least two years of recent clinical nursing experience in hospice or medical/surgical nursing
- Preferred experience in home health care, hospice, or oncology
- Bilingual in Spanish and English is advantageous
Compensation: $74,000 - $105,000
