Compensation: The salary for this role ranges from $75,000 to $98,000 annually, based on education, experience, and certifications. Full-time positions come with benefits including health insurance, a 401k plan with profit sharing, paid time off, and additional perks.

Position Details:

  • Job Title: Hospice Nurse
  • Shift: 12.5-hour shifts, three days per week, including two weekends per month
  • Reports To: Director of Clinical Services

Qualifications:

  • Current Registered Nurse license in New York State
  • Ability to engage effectively with a diverse population
  • One to two years of recent bedside nursing experience with strong clinical skills; at least two years in Med-Surg nursing
  • Experience with terminally ill patients and a commitment to hospice care
  • Preferred experience in home care, hospice, and/or oncology
  • Strong assessment skills to address the needs of patients and families
  • Knowledge of medications and therapies for pain and symptom management
  • Effective communication skills, both verbal and written
  • Willingness to obtain hospice and palliative care nursing certification within three years of employment
  • Good listening abilities and teamwork orientation
  • Proficiency in computer use and Microsoft Windows

Physical Requirements:

  • Physical agility for tasks such as walking, bending, and lifting over 50 lbs
  • Ability to perform data entry and use a computer for extended periods

Cognitive Requirements:

  • Active participation in coordinating patient care according to professional standards
  • Ongoing interdisciplinary assessment and treatment planning
  • Capability to execute various treatments and procedures

Key Responsibilities:

  • Deliver direct care to residents following nursing standards and facility policies.
  • Administer medications and document interventions accurately.
  • Conduct thorough resident assessments and maintain detailed documentation.
  • Oversee the admission process for new residents.
  • Collaborate with the Interdisciplinary Team to develop and update care plans.
  • Perform ongoing assessments of resident and family needs, implementing appropriate interventions.
  • Maintain proficiency with medical equipment and intravenous access devices.
  • Ensure accurate and timely documentation in medical records.
  • Work collaboratively with clinical and support staff to coordinate daily care.
  • Communicate changes in residents' conditions to the medical team.
  • Participate in quality assurance programs and training.
  • Facilitate smooth transitions for resident transfers.
  • Engage in family conferences as needed.
  • Continuously evaluate the quality of care and resident satisfaction.
  • Assist in training new staff members.
  • Embrace the organization's commitment to customer service and quality improvement.
