Compensation: The salary range for this position typically falls between $52,000 and $57,000 annually, based on relevant experience and qualifications. We provide outreach, education, and direct support services to populations facing significant health disparities, including low-income individuals, those with chronic illnesses, and marginalized communities.

Role Summary: The HIV Prevention Coordinator is responsible for implementing various HIV prevention initiatives. Key responsibilities include:

  • Conducting outreach to engage individuals at high risk for HIV infection.
  • Providing education on behavioral practices and medical interventions that can reduce the risk of HIV acquisition.
  • Facilitating intake assessments and connecting clients to pre-exposure prophylaxis (PrEP) and post-exposure prophylaxis (PEP) services.

This role involves working closely with high-risk HIV-negative clients, particularly those who are substance users. The Coordinator will supervise outreach and education staff, ensuring effective service delivery and client engagement.

Key Responsibilities:

  • Participate in community outreach initiatives to build relationships with potential clients.
  • Conduct screenings to identify individuals at high risk for HIV.
  • Enroll eligible clients in prevention programs and develop personalized service plans.
  • Assist clients in executing their service plans, including referrals and transportation to appointments.
  • Educate clients on prevention strategies, including the importance of regular HIV testing and safe practices.
  • Provide information and linkage to PrEP and PEP services, ensuring follow-up support.
  • Maintain communication with clients who miss appointments to encourage adherence to their service plans.
  • Supervise outreach and education staff, providing training and support as needed.
  • Document client interactions and maintain accurate records of outreach efforts.
  • Collaborate with internal teams and external partners to enhance service delivery.

Qualifications:

  • A minimum of 3 years of relevant experience is required; a Bachelor's degree may substitute for some experience.
  • A high school diploma or equivalent is mandatory.
  • At least 1 year of supervisory or program management experience is preferred.
  • Experience in community outreach is essential.
  • Fluency in Spanish is highly desirable.
  • Knowledge of HIV, HCV, STI, and harm reduction strategies is required.
  • Strong organizational and communication skills are necessary.
