About the Role

**Job Summary:**

We are seeking a highly skilled and experienced professional to lead our HIV Care Coordination Program as a Client Services Coordinator Lead. This is a critical role that requires strong leadership, communication, and problem-solving skills to manage a team of navigators and ensure the successful implementation of our program.

Key Responsibilities:

  • Program Management: Oversee the daily operations of the HIV Care Coordination Program, including case finding, client recruitment, and enrollment processes.
  • Team Leadership: Supervise and coach a team of navigators to ensure they are meeting program goals and objectives.
  • Client Services: Provide high-quality client services, including comprehensive assessments, service planning, and ongoing support to clients.
  • Collaboration: Work closely with the medical team, patient navigators, and other stakeholders to ensure seamless coordination of care.
  • Quality Assurance: Conduct quality assurance reviews and participate in quality improvement projects to ensure the program is meeting its goals and objectives.
  • Reporting: Track and report on program performance metrics, including client engagement, retention, and health outcomes.

Requirements:

  • Experience: At least three years of direct service experience in HIV care management, with at least one year of supervisory experience.
  • Education: Bachelor's degree preferred, high school or equivalent required.
  • Skills: Strong leadership, communication, and problem-solving skills, with the ability to work independently and as part of a team.
  • Knowledge: In-depth knowledge of HIV disease progression, treatments, and adherence strategies, as well as care management principles and documentation.
