About the Role

We are seeking a highly motivated and compassionate HIV Patient Navigator to join our team at Community Health Action of Staten Island (CHASI). As a key member of our healthcare team, you will play a critical role in facilitating access to care and services for individuals living with HIV.

Key Responsibilities
  • Provide personalized support and advocacy to clients, accompanying them to medical appointments and community agencies as needed.
  • Coordinate logistics for care and treatment needs, including appointment reminders, transportation, and childcare arrangements.
  • Deliver individualized health education to clients, addressing their unique needs and concerns.
  • Assess treatment adherence and address barriers to care, ensuring clients receive optimal support.
  • Participate in care planning activities, collaborating with healthcare professionals to develop comprehensive treatment plans.
  • Conduct modified Directly Observed Therapy (mDOT) services, providing critical support to clients in need.
  • Communicate effectively with service providers, providing critical feedback and insights to inform care delivery.
  • Engage in ongoing professional development, meeting training requirements and adhering to documentation standards.
Requirements
  • High school diploma or equivalent, with related experience and strong socio-cultural identification with the target population or certification as a peer worker with at least 12 months experience in patient navigation.
  • At least 1 year of experience in case management and/or patient navigation.
  • Bilingual (English/Spanish) preferred.
Competencies
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, colleagues, and community partners.
  • Strong computer literacy, with proficiency in Microsoft products and ability to navigate the internet.
  • Knowledge of HIV progression, treatments, and adherence strategies, with the ability to communicate complex information effectively.
  • Ability to work independently and as part of a team, with a strong commitment to collaboration and teamwork.
  • Strong problem-solving and critical thinking skills, with the ability to analyze complex situations and develop effective solutions.
