Job Overview:
Horizon House, Inc. is a well-respected provider of services aimed at assisting adults with various support needs stemming from developmental disabilities, behavioral health challenges, and substance use disorders. Key Responsibilities:
  • Facilitate access to essential services that enable individuals to thrive socially, educationally, and professionally within the community.
  • Identify and maintain regular support systems for clients to ensure continuity of care.
  • Ensure a seamless delivery of services in alignment with contractual obligations and agency regulations.

Supervision Structure:
  • Direct supervision will be provided by the Program Director or designated representatives.

Additional Duties:
  • Oversee the design and availability of appropriate services for clients, including managing the referral process and ensuring compliance with regulatory standards.
  • Participate in developing and refining individualized rehabilitation plans that incorporate social, educational, vocational, self-care, and residential objectives.
  • Assess service delivery gaps and propose alternative action plans as necessary.

Qualifications:
  • A Bachelor's degree in Human Services, Social Science, Healthcare, or a related field, along with three years of direct care experience; or a combination of education and experience that provides the necessary knowledge and skills.
  • Excellent communication and organizational skills.
  • Ability to work both independently and collaboratively within a focused team.
  • Must possess a valid driver's license and have an acceptable driving record for local travel.

Physical Requirements:
  • Frequent verbal communication and active listening.
  • Physical ability to lift, carry, kneel, reach, stack, stoop, and bend, which may involve direct client intervention.
  • May require crisis intervention and emergency assistance in urgent situations.
  • Flexibility in scheduling and potential on-call responsibilities are essential.
  • Must be prepared to report to work during emergencies, including inclement weather.
  • Manual dexterity and fine motor skills are necessary.
