JOB TITLE: Client Support Specialist

DEPARTMENT: Archdiocese of Philadelphia, McAuley House

Housing and Homeless Services Division

REPORTS TO: Program Supervisor

DIRECT REPORTS: None

SUMMARY:

As a Client Support Specialist at the Archdiocese of Philadelphia, McAuley House, you will deliver comprehensive case management services to individuals enrolled in the Residential Program, adhering to established social service protocols.

KEY RESPONSIBILITIES:

  • Facilitate the intake process and provide orientation for new clients.
  • Conduct thorough assessments for assigned individuals.
  • Develop tailored Individual Service Plans with specific long-term objectives.
  • Assist clients in scheduling and attending medical, mental health, and other necessary appointments.
  • Maintain accurate client medical records and medication distribution logs.
  • Regularly document client progress and relevant information.
  • Monitor client health, medication adherence, and treatment plans.
  • Provide one-on-one counseling and lead group sessions.
  • Stay informed about available resources and referral options for clients.
  • Engage in ongoing training and participate in team meetings.
  • Adapt work hours to meet the needs of various client activities.
  • Carry out additional responsibilities as assigned.

QUALIFICATIONS:

  • At least two years of experience in social services.
  • Proficient in computer applications.
  • Knowledgeable about issues related to homelessness, substance use, and mental health.
  • Demonstrates compassion and commitment to serving vulnerable populations.

EDUCATIONAL REQUIREMENTS:

  • Bachelor's degree in Social Work or a related field.

EXPERIENCE REQUIREMENTS:

  • Minimum of two years of experience in social work.
  • Possession of a valid Driver's License.

PHYSICAL DEMANDS:

  • Ability to stand, sit, walk, climb stairs, operate controls, reach with hands, and occasionally lift items weighing up to 40 lbs.
