Job Summary:

North American Staffing Group is seeking a highly skilled Licensed Vocational Nurse (LVN) to fill the role of LVN Case Manager - Home Care Coordinator. This position will be responsible for coordinating home care services for program participants, including the development and implementation of care plans, coordination of durable medical equipment, and nursing home care.

Key Responsibilities:
  • Assess the home care needs of a frail elderly population and develop specific plans of care.
  • Collaborate with the Interdisciplinary team to evaluate and re-evaluate caregiver hours and assignment plans.
  • Communicate with participants, family members, and caregivers to discuss and educate on approved caregiver hours and assignment plans.
  • Act as a liaison between Homecare vendors and PACE related to participant needs.
  • Maintain patient confidentiality and comply with federal, state, and local legal and professional requirements.
Requirements:
  • Graduation from an accredited LVN school and current LVN license issued by the State of California.
  • A minimum of one (1) year experience practicing as a Licensed Vocational Nurse.
  • Current CPR/BLS certification.
  • Experience and knowledge regarding the physical, mental, and social needs of the patients.
  • Valid California driver's license with a good driving record.
Preferred Qualifications:
  • Bilingual: English/Spanish/Mandarin/Cantonese depending on location.
  • Ability to work well with patients, requiring patience and a kind/caring attitude.
  • Ability to work effectively in a team environment.
