Choice Health Group is actively seeking a dedicated LVN Case Manager to enhance our dynamic team. The selected candidate will be responsible for overseeing the care plan to ensure it aligns with the admission data, ongoing assessments, age-specific care, cultural considerations, and appropriate patient acuity.
Key Responsibilities:
- Facilitates the effective formulation and execution of discharge plans, addressing any barriers and providing education to patients and their families.
- Engages with patients and their significant others to discuss the care plan.
- Tracks the patient's progress in accordance with their individualized care plan.
- Adjusts the care plan continuously based on the patient's condition and progress evaluations in collaboration with the clinical team.
- Addresses patient inquiries regarding medication purposes, potential side effects, and administration guidelines.
Qualifications:
- Completion of an accredited practical nursing or vocational nursing program.
- A minimum of one to two years of clinical case management experience; experience in community health, hospice, or medical/surgical settings is preferred.
- Current licensure as an LPN/LVN in the state.
- Adherence to accepted professional standards and practices.
- Strong verbal and written communication skills, along with effective organizational abilities.
Please note that this position requires on-call duties, which are scheduled on weekends and rotate 1-2 times per month. Specific weekends will be assigned, with responsibilities including responding to calls during designated hours.
