Choice Health Group is actively seeking a dedicated LVN Case Manager to become an integral part of our expanding team. The selected LVN will oversee the care plan to ensure it accurately reflects the admission data, ongoing assessments, age-appropriate care, cultural considerations, and suitable acuity levels.
Key Responsibilities of the LVN Case Manager:
- Facilitates the effective development and execution of discharge plans, addressing any barriers and providing education to patients and their families.
- Engages with patients, families, and significant others to discuss the care plan.
- Monitors patient progress in alignment with their care plan.
- Continuously revises the care plan based on patient condition and progress evaluations in collaboration with the clinical team.
- Responds to patient inquiries regarding medication purposes, potential side effects, and administration guidelines.
Qualifications for the LVN Case Manager:
- Graduate of an accredited practical nursing or vocational nursing program.
- A minimum of one to two years of clinical case management experience; experience in community health, hospice, or medical/surgical settings is preferred.
- Current licensure as an LPN/LVN in the relevant state.
- Adheres to accepted professional standards and practices.
- Exhibits strong verbal and written communication skills, along with effective organizational abilities.
Additionally, this position requires on-call availability, typically on weekends, with a rotation of 1-2 times per month. Specific weekends will be assigned, and duties will include responding to calls during designated hours. The on-call compensation will be discussed during the interview process.
