Job Summary

We are seeking a highly skilled LVN Case Manager to join our team at the Los Angeles LGBT Center. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and ensuring the highest level of service delivery.

Key Responsibilities
  • Collaborate with healthcare providers to develop and implement comprehensive care plans that meet the unique needs of our patients.
  • Provide case management services to patients living with HIV/AIDS, ensuring they are aware of available services and resources.
  • Work closely with RN Care Managers and the Medical Care Coordination team to assess, implement, and maintain programs under DHSP guidelines.
  • Coordinate referrals, authorizations, billing, and contract requirements to ensure seamless service delivery.
  • Develop and maintain clear and concise communication with patients, families, and healthcare providers to ensure continuity of care.
  • Maintain accurate and up-to-date patient records, including case presentations, progress notes, medication lists, and summaries.
  • Coordinate services with outside agencies, such as home health, hospital admissions, and hospice care, to ensure comprehensive patient support.
  • Provide health education and promotion to patients, partners, and family members, reinforcing education from medical providers.
  • Identify and eliminate barriers to medical services, promoting early and ongoing interventions and adherence.
  • Interact professionally with healthcare staff and patients, exercising sound judgment and decision-making skills.
  • Assist with data auditing tasks, prepare reports, and submit them in a timely manner.
  • Participate in staff meetings, agency meetings, planning meetings, and other meetings as needed.
  • Comply with OSHA, HIPAA, and other applicable regulations.
Requirements
  • Knowledge of or experience working with the LGBTQ community and familiarity with issues relevant to LGBTQ people.
  • Passion for the Center's mission to make the world a better place for LGBTQ people.
  • Current California LVN license.
  • Current AHA CPR certification.
  • Minimum 3 years of experience in direct patient care, preferably in an ambulatory care setting.
  • Minimum 2 years of experience in Case Management.
  • Experience in HIV/AIDS, other infectious diseases, and chronic disease care/management, preferred.
  • Knowledge of quality assurance/management/improvement, as well as DHSP and other regulatory standards.
  • Excellent verbal and written communication skills, including strong organizational, detail, and interpersonal skills.
  • Excellent computer skills and knowledge of Microsoft Office, word processing, Electronic Health Records, and other software systems.
  • Ability to be self-motivated and dedicated to initiating and completing complex tasks.
  • Ability to handle multiple tasks, be detail-oriented, and maintain strict confidentiality of medical information.
  • Able to work well independently with minimal supervision.
  • Bilingual skills, a plus (English/Spanish fluency preferred).
  • Demonstrated ability to work effectively with people of diverse backgrounds in a multicultural environment.
