Job Summary

The Outpatient Case Manager is a critical role within Regal Medical Group, Inc. responsible for ensuring the seamless coordination of patient care across various healthcare settings. This position requires a skilled professional with a strong background in nursing and case management to develop and implement effective care plans that meet the unique needs of each patient.

Key Responsibilities
  • Assessment and Planning: Conduct thorough assessments of patients to identify their medical, environmental, and psychosocial needs, and develop individualized care plans that address these needs.
  • Care Coordination: Collaborate with healthcare providers, patients, and families to ensure that patients receive the necessary care and services to achieve optimal health outcomes.
  • Transition Management: Identify and address potential barriers to care, and develop strategies to facilitate smooth transitions between healthcare settings.
  • Communication and Education: Communicate effectively with patients, families, and healthcare providers to ensure that all parties are informed and engaged in the care planning process.
  • Documentation and Reporting: Maintain accurate and timely documentation of patient care activities, and provide regular reports to healthcare providers and other stakeholders as needed.
Requirements
  • Education and Experience: Graduate from an accredited Registered Nursing Program or Licensed Vocational Nursing Program, with a minimum of 3 years of acute care or case management experience.
  • License and Certification: Current CA RN or LVN license, current CPR certification, and valid CA Driver's license.
  • Skills and Knowledge: Strong knowledge of healthcare systems, policies, and procedures, as well as excellent communication, problem-solving, and organizational skills.
