The Outpatient Case Manager RN is a key member of our healthcare team, responsible for providing high-quality patient care and coordination of services. This role requires strong clinical skills, excellent communication, and a patient-centered approach.
Key Responsibilities- Assessment and Planning
- Conduct thorough assessments of patients to identify their medical, environmental, and psychosocial needs.
- Develop and implement individualized care plans that address patient needs and goals.
- Coordination of Care
- Collaborate with healthcare providers, patients, and families to ensure seamless care coordination.
- Communicate effectively with patients, families, and healthcare teams to ensure understanding and adherence to care plans.
- Transition of Care
- Identify and address potential transitions of care, including hospitalizations, readmissions, and changes in patient status.
- Develop and implement strategies to reduce unplanned transitions of care.
- Community Resources
- Identify and connect patients with community resources that address their medical, environmental, and psychosocial needs.
- Facilitate access to community resources and services.
- Documentation and Reporting
- Accurately and thoroughly document patient assessments, care plans, and progress.
- Provide regular reporting to healthcare teams and stakeholders on patient progress and outcomes.
- Education and Experience
- Graduate from an accredited Registered Nursing Program or Licensed Vocational Nursing Program.
- Current CA RN, or LVN license, current CPR certification, valid CA Driver's license.
- 3 years acute care or case management experience preferred.
- 2-3 years of utilization or HMO experience preferred.
- Skills and Abilities
- Strong clinical skills and knowledge of healthcare systems.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Strong problem-solving and critical thinking skills.
