Job Summary

The Outpatient Case Manager RN/LVN is a critical role within Regal Medical Group, Inc. responsible for ensuring the highest level of care for our patients. Key Responsibilities
  • Assessment and Planning: Conduct thorough assessments of patients to identify their healthcare needs and develop individualized care plans.

  • Communication: Effectively communicate with patients, families, and healthcare teams to ensure that patient needs are met and that care plans are implemented.

  • Community Resources: Identify and connect patients with community resources to support their healthcare needs.

Requirements
  • Education: Graduate from an accredited Registered Nursing Program or Licensed Vocational Nursing Program.
  • Experience: 3 years acute care or case management experience preferred.
  • Licensure: Current CA RN, or LVN license, current CPR certification, valid CA Driver's license.
  • Skills: Typing 40 words per minute with accuracy, knowledge of computers, faxes, printers, and all other office equipment, knowledgeable in MS Office Programs (i.e., Word, Excel, Outlook, Access, and PowerPoint).
