The Outpatient Case Manager RN/LVN is a critical role within Regal Medical Group, Inc. responsible for ensuring the highest level of care for our patients. As a key member of our healthcare team, you will be responsible for assessing, planning, implementing, and evaluating patient care to achieve optimal health outcomes.
Key Responsibilities- Assessment and Planning: Conduct thorough assessments of patients to identify their healthcare needs and develop individualized care plans.
- Care Coordination: Collaborate with healthcare providers, patients, and families to ensure seamless transitions of care and optimal health outcomes.
- Communication: Effectively communicate with patients, families, and healthcare teams to ensure that patient needs are met and that care plans are implemented.
- Documentation: Accurately document patient information and care plans in accordance with regulatory requirements and company policies.
- Community Resources: Identify and connect patients with community resources to support their healthcare needs.
- Quality Improvement: Participate in quality improvement initiatives to enhance patient care and outcomes.
- Education: Graduate from an accredited Registered Nursing Program or Licensed Vocational Nursing Program.
- Experience: 3 years acute care or case management experience preferred.
- Licensure: Current CA RN, or LVN license, current CPR certification, valid CA Driver's license.
- Skills: Typing 40 words per minute with accuracy, knowledge of computers, faxes, printers, and all other office equipment, knowledgeable in MS Office Programs (i.e., Word, Excel, Outlook, Access, and PowerPoint).
