Job Summary

Regal Medical Group, Inc. is seeking a highly skilled Inpatient Case Manager RN/LVN to join our team. As a key member of our clinical staff, you will be responsible for providing exceptional patient care and ensuring seamless transitions between hospital and post-acute settings.

Key Responsibilities
  • Assessment and Planning: Conduct thorough assessments of patients to identify their needs and develop individualized care plans.
  • Care Coordination: Collaborate with healthcare teams to ensure timely and effective delivery of care services.
  • Discharge Planning: Develop and implement safe discharge plans to ensure patients receive necessary resources and support.
  • Communication: Maintain open and effective communication with patients, families, and healthcare providers to ensure seamless care transitions.
  • Documentation: Accurately and thoroughly document patient information and care plans in accordance with regulatory and company guidelines.
Requirements
  • Education: Graduate from an accredited Registered Nursing Program or Licensed Vocational Nursing Program.
  • Experience: At least 2 years of clinical experience in the acute care setting.
  • Skills: Strong communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
  • Certifications/Licenses: Current California License, with RN or LVN certification.
