About the Role

We are seeking a highly skilled Pharmacy Navigator I to join our team at Hackensack Meridian Health. As a Pharmacy Navigator I, you will play a critical role in ensuring patients receive their prescriptions smoothly and efficiently.

Key Responsibilities
  • Establish initial greetings to patients admitted or with clinic appointments, explaining services available via HMH Pharmacies.
  • Perform prescription order entry duties as necessary.
  • Obtain patient authorization to process prescriptions through the ambulatory or specialty pharmacy.
  • Engage in collaborative processes with floor nurses, discharge planners, case managers, social workers, physicians, APNs, PAs, medical residents, unit clerks, and other teams/initiatives to ensure patients scheduled for discharge or with clinic appointments can receive critical medications in a timely manner.
  • Work to determine patients being scheduled for discharge or with clinic appointments to ensure HMH Pharmacies are selected by patient choice to provide services.
  • Determine information sources necessary to process prescriptions (e.g., insurance coverage).
  • Identify and troubleshoot patient medication access issues related to insurance, financial, or insurance coverage and help resolve them so patients can receive needed medications.
  • Adhere to all established HMH Pharmacy and Federal/State privacy and compliance policies to protect patient confidentiality.
  • Ensure prescription fulfillment by in-house pharmacy based on priority needs to allow for timely discharge and/or start of specialty medication.
  • Coordinate and facilitate patient pickup of prescriptions and/or delivery of completed prescription orders to patient care areas or home.
  • Ensure patients receive an offer for pharmacist counseling and distribute pharmacist contact information in the event patients have prescription-related questions at home.
  • Facilitate distribution of medication-related educational materials and coordinate counseling with staff pharmacist where needed.
  • Assist Ambulatory Pharmacy staff as needed.
  • Perform duties of Ambulatory Pharmacy Technician if required.
  • Ensure all guests/patients receive safe care. Maintain a 'safety first' work ethic. Report any medication errors or medication variances as required by HMH Pharmacy Policy.
  • Other duties and/or projects as assigned.
  • Adhere to HMH Organizational competencies and standards of behavior.
Requirements
  • High School diploma, general equivalency diploma (GED), and/or GED equivalent programs.
  • Excellent written and verbal communication skills.
  • Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.
Preferred Qualifications
  • Retail/Specialty Pharmacy experience.
  • Strong knowledge and experience with PBM Claims and coverage.
Licenses and Certifications
  • Pharmacy Technician License.
  • PTCB or Equivalent Certification Required within 180 days of start of employment with the following exceptions:
    • Hired prior to January 2024.
    • Actively enrolled in ACPE accredited school of pharmacy / another allied health program or hired before.
Work Environment
  • Regular contact with medical personnel and its visitors.
