Job Summary:

We are seeking a highly skilled and experienced Nurse Manager - Client Services to join our team at LifePath Inc. This is a full-time, hybrid role based in Greenfield, with a competitive salary range of $70k - $75K, and excellent benefits.

About the Role:

The Nurse Manager - Client Services will be responsible for the administrative and clinical oversight of programs that complete assessments and redeterminations for individuals seeking community-based programs and eligibility for skilled nursing facilities. This role will directly supervise Home Care nurses and serve as a resource for all Client Services programs, including Adult Family Care (AFC) and Personal Care Attendant (PCA).

Key Responsibilities:

  • Provides oversight of the Home Care Nursing Program, including clinical assessment and eligibility determinations for Nursing Facility and Community-Based programs.
  • Works closely with Program Directors and Supervisors of the PCA and AFC program to provide clinical and administrative oversight of nursing functions.
  • Provides consultation to all Client Services departments regarding clinical issues.
  • Participates in service coordination and case conferences with clients, their families, LifePath staff, community agencies, nursing facilities, and other health/community professionals as needed.
  • Conducts quality assurance/quality improvement activities for the Home Care Nursing function and assists QA/QI for nursing functions within the PCA and AFC programs.
  • Works closely with other Client Services Program Directors on policies and procedures and operations involving the nursing function in departmental programs.
  • Provides quarterly orientation and refreshers to new and existing staff regarding the role of the Home Care nursing team.
  • Supervises Home Care RNs, participates in the interviewing and hiring process, and conducts employee performance evaluations, individual monthly supervisions, and team meetings.
  • Coordinates regular meetings of all nurses to provide education, training, and peer support.
  • Interfaces with community health and human services providers.
  • Attends required meetings and trainings.
  • Maintains documentation and completes reports as required by agency and program regulations.

Requirements:

  • Registered Nurse with a valid, current Massachusetts RN license.
  • BSN from an accredited nursing school and three years of clinical experience in a community setting or long-term care setting, and at least one year of supervisory, administrative, or managerial experience.
  • Strong organizational skills, including the ability to manage time effectively, prioritize tasks, set goals, and develop systems for achieving those goals.
  • Highly adept in organizing complex planning processes involving diverse groups of stakeholders.
  • Knowledge of key drivers of quality assurance, quality improvement, and quality enhancement initiatives.
  • Professional verbal and written communication skills, with good listening and engagement skills.
  • Ability to work well across disciplines and with diverse teams.
  • Values each staff member and their contributions, providing feedback on performance and assisting staff to develop to their fullest.
  • Influences others through positive role modeling, assists with training others, and acts as an ambassador to assigned programs and the greater agency.
  • Ability to navigate complex social and healthcare systems impacting older adults and individuals with disabilities.
  • Self-motivated and proven ability to meet short-term and long-term goals.
  • Intermediate to advanced practical application of MS Word, Excel, and Google suite skills, with competence in virtual platforms such as Zoom/Google Meet, and the ability to manage electronic databases and other electronic platforms.

Working Conditions:

The Nurse Manager - Client Services will work in a fast-paced, rapidly changing environment, with a split between indoor office work and traveling outdoors to home visits to clients. The role requires the ability to tolerate odors and pollutants, including smoke, air fresheners, pet dander, personal odors, etc. from a variety of in-home conditions, as well as the by-products of office machine operation, and seasonal changes and indoor temperatures.
