Job Summary:
We are seeking a highly skilled and experienced Nurse Manager - Client Services to join our team at LifePath Inc. This is a full-time, hybrid role based in Greenfield, with a starting salary of $70k - $75K, and excellent benefits.
Key Responsibilities:
- Provide administrative and clinical oversight of programs that complete assessments and redeterminations for individuals seeking community-based programs and skilled nursing facilities.
- Directly supervise Home Care nurses and serve as a resource for all Client Services programs, including Adult Family Care (AFC) and Personal Care Attendant (PCA).
- Participate in interdisciplinary case management and coordinate nursing teams across the agency.
- Collaborate with Program Directors and Supervisors to provide clinical and administrative oversight of nursing functions.
- Consult with Client Services departments on clinical issues and participate in service coordination and case conferences.
- Conduct quality assurance/quality improvement activities for the Home Care Nursing function and assist with QA/QI for nursing functions within the PCA and AFC programs.
- Develop and implement policies and procedures related to nursing functions in departmental programs.
- Provide quarterly orientation and refreshers to new and existing staff on the role of the Home Care nursing team.
- Supervise Home Care RNs, participate in interviewing and hiring, and conduct employee performance evaluations.
- Coordinate regular meetings of all nurses to provide education, training, and peer support.
- Interface with community health and human services providers and attend required meetings and trainings.
- Maintain documentation and complete reports as required by agency and program regulations.
Requirements:
- Registered Nurse with a valid, current Massachusetts RN license.
- BSN from an accredited nursing school and three years of clinical experience in a community setting or long-term care setting.
- At least one year of supervisory, administrative, or managerial experience.
Working Conditions:
The Nurse Manager - Client Services will work in a fast-paced, rapidly changing environment, with a split between indoor office work and traveling outdoors to home visits. The role requires the ability to tolerate odors and pollutants, heat and cold, and the physical demands of driving and navigating uneven terrain.
