Responsibilities
Program Overview

The MYPAC initiative, developed by the Mississippi Division of Medicaid, aims to provide essential support to families and youth in their home environments.

Participants are referred based on mental health diagnoses and the family's challenges in maintaining the child at home.

Position Overview
  • Manage a caseload of 10-12 youth and families, delivering comprehensive care coordination services.
  • Facilitate collaborative meetings to ensure wraparound support.
  • Work closely with families and key stakeholders to create and refine the Individual Service Plan.
  • Develop and update the Individualized Crisis Management Plan as necessary.
  • Provide 24/7 on-call crisis support.
  • Assess crisis calls to determine the most appropriate response.
  • Conduct care coordination activities as outlined in wraparound meetings.
  • Offer transportation to appointments and activities when families lack resources.
  • Oversee medication administration and ensure compliance.
Additional Information

The work schedule is flexible and designed around the availability of the youth and families served.

Applicants must possess a valid driver's license, have access to a personal vehicle for work-related purposes, and maintain auto insurance.

Community-based staff will receive reimbursement for applicable mileage.

Salary:
Compensation ranges from $46,000 to $53,000 annually, depending on educational background and clinical licensure.Qualifications
Requirements
  • A Bachelor's degree in a social services discipline is required, with experience working with at-risk youth, adults, or families.
  • A Master's degree in a relevant field is preferred.
  • Acceptable degrees include social work, drug and alcohol counseling, psychology, criminal justice, guidance counseling, or marriage and family therapy.
  • Other degrees will be evaluated to determine eligibility.
  • Strong organizational skills and meticulous attention to detail.
  • Excellent written, verbal, and interpersonal communication skills.
  • Ability to manage multiple priorities effectively.
  • Basic computer proficiency.
  • Flexibility in scheduling is essential.
