Service Coordination Specialist – Nichols College

Responsibilities
Program Overview

The Mississippi Division of Medicaid has initiated the MYPAC program to support troubled children and their families in their home environments.

Referrals are made for youth facing mental health challenges and for families struggling to maintain the child at home.

Position Overview
  • Manage a caseload of 10-12 youth and families, providing essential care coordination services.
  • Facilitate wraparound meetings to ensure comprehensive support.
  • Collaborate with families and key stakeholders to create and refine the Individual Service Plan.
  • Develop and update the Individualized Crisis Management Plan as necessary.
  • Be available for on-call crisis support 24/7.
  • Triage crisis calls to assess the appropriate level of assistance required.
  • Complete care coordination tasks identified during wraparound meetings.
  • Provide transportation to appointments and activities when families lack resources.
  • Monitor medication administration to ensure compliance.
Additional Information

The schedule is flexible and non-traditional, designed around the availability of the youth and families served.

Applicants must possess a valid driver's license, a personal vehicle for work-related purposes, and auto insurance.

Community-based staff will receive reimbursement for applicable mileage.

Salary:
$46,000 – $53,000 per year, commensurate with education and clinical licensure.
Qualifications
Requirements
  • A Bachelor's degree in a social services discipline with experience working with at-risk youth, adults, or families is required.
  • A Master's degree in a relevant social services field is preferred.
  • Acceptable degrees include social work, drug and alcohol education or counseling, psychology, criminal justice, guidance counseling, or marriage and family therapy.
  • Other degrees will be evaluated to determine their suitability.
  • Strong organizational skills and meticulous attention to detail.
  • Excellent written, verbal, and oral communication skills.
  • Ability to manage multiple priorities effectively.
  • Basic computer proficiency.
  • Flexibility in scheduling is essential.
