Client Support Specialist

The Client Support Specialist will demonstrate excellent communication abilities and a collaborative spirit.
This role involves assessing the individual needs of each resident, focusing on various aspects such as mental health, housing stability, financial well-being, health and wellness, educational opportunities, vocational training, and social engagement.
Strategic Planning:
In partnership with other team members, you will create and execute a tailored plan to meet the identified needs.
Resource Linkage:
Your responsibilities will include connecting residents with essential services and support systems, both internal and external, to fulfill the objectives of the care plan.
Monitor residents' progress and make necessary adjustments to the plan as required.
Advocacy:
Act as an advocate for residents, ensuring they access and utilize the services they need.
Provide ongoing support throughout the case management process, employing techniques from Motivational Enhancement Therapy and a strengths-based approach.
Engage in outreach efforts to discover vital resources.
Offer individualized coaching and support to residents.
Encourage participation in recovery-oriented activities.
Assist residents in identifying and accessing necessary resources and recovery supports.
Facilitate discharge planning, collaborating with other community resources.
Participate in team meetings, community discussions, and program evaluations.
Ensure timely data collection and report completion.
