Key Responsibilities:
- Provide individualized support to residents, promoting independence and self-advocacy
- Develop and implement personalized care plans, ensuring the unique needs of each resident are met
- Collaborate with the multidisciplinary team to ensure comprehensive care and support
- Assist with daily living activities, including medication management and personal care
- Document resident progress and provide regular updates to the care team
Requirements:
- High school diploma or equivalent required; degree in a related field preferred
- Experience working with individuals with intellectual and developmental disabilities
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
