About the Role

Ferry County Public Hospital District 1 is seeking a highly skilled and compassionate Mobile Health Specialist to join our team. As a key member of our multi-disciplinary team, you will provide a variety of care in the community and home settings, improving patient health outcomes and preventing re-admissions to the Emergency Room, Rural Health Clinic, and Hospital level of care.

Key Responsibilities
  • Deliver patient care plans and orders as prescribed by the Provider
  • Administer patient care following established active protocols
  • Document care provided, assessments, and phone communication events in the electronic medical record
  • Manage the referral dashboard in the electronic health record and accurately update patient statuses
  • Maintain knowledge of Ferry County Public Hospital District 1 nursing district policies
  • Communicate closely with external care partners to achieve timely social, medical, and behavioral health referrals as needed
Essential Duties and Responsibilities
  • Deliver patient care plans and orders as prescribed by the Provider
  • Administer patient care following established active protocols
  • Document care provided, assessments, and phone communication events in the electronic medical record
  • Manage the referral dashboard in the electronic health record and accurately update patient statuses
  • Maintain knowledge of Ferry County Public Hospital District 1 nursing district policies
  • Communicate closely with external care partners to achieve timely social, medical, and behavioral health referrals as needed
  • Provide a compassionate and calming environment for patients so any questions or concerns can be adequately addressed
  • Deliver care in the home setting to patients identified as homebound with an active Rural Health Clinic care plan
  • Inspect, clean, and restock the MIH truck to ensure that all equipment and supplies are present, in good operating order, and ready for response
  • Drive the MIH truck safely as a representative of the district while also following all Washington State statutes and revised codes
Professional Requirements
  • Actively participate in departmental meetings and discussions
  • Complete all mandatory education within the given timeframe
  • Maintain regulatory requirements
  • Report to work on time and as scheduled
  • Wear identification while on duty; use computerized punch time system accurately
  • Represent the organization in a positive and professional manner in the community
  • Attend annual review and department in-services as scheduled
  • Attend at least 75% of Nursing Department staff meetings; review all monthly staff meeting minutes
  • Actively participate in the department's quality improvement (QI) activities
  • Comply with all organizational policies regarding ethical business practices and the employee handbook
  • Communicate the mission, vision, and values of the organization as well as the focus of the department
  • Meet dress code standards for the department; appearance is neat and clean
Job Knowledge & Qualifications
  • Education and/or Work Experience Requirements
  • Graduate of an accredited school of nursing
  • Current RN License in WA State
  • Current AHA BLS
  • Current AHA PALS
  • Current AHA ACLS
  • PHTLS preferred, or active TNCC certification required
  • Current WA State Driver's License
