Job Overview

We are seeking a highly motivated individual to join our team as a Family Care Coordinator. This role plays a critical part in the further development of the family-driven care movement within Onondaga County.

The successful candidate will have a strong background in navigating the mental health system, with experience working collaboratively with various stakeholders. A credentialled Family Peer Advocate through NYS is highly preferred.

Key Responsibilities
  • Assist in the development and implementation of projects aimed at enhancing service delivery to youth with co-occurring disorders.
  • Build and maintain relationships with community-based agencies providing Home and Community Based Services Medicaid Waiver and NYS OMH Child and Family Treatment and Support Services (CFTSS).
  • Design and implement strategies to increase leadership skills among caregivers.
Requirements
  • A high school diploma or equivalent is required, while an associate degree is preferred.
  • Lived experience navigating the system as a primary caretaker for a child with mental health needs is highly valued.
  • Credentialed as a Family Peer Advocate through NYS with two years' experience providing peer support services is preferred.
Estimated Salary Range:$48,000-$52,000 yearly.Benefits
