Care Coordinator Specialist

Menorah Park, located in Syracuse, New York, is seeking a full-time LPN Care Coordinator to join their team. This role involves coordinating and managing the care of residents, ensuring they receive high-quality care. The LPN Care Coordinator will work closely with the healthcare team to develop personalized care plans and ensure all medical needs are met.
This position requires excellent communication and organizational skills to manage multiple responsibilities effectively. Strong interpersonal skills are essential for building relationships with residents, families, and healthcare professionals.

Responsibilities:
- Develop and implement safe, efficient, and therapeutic care plans.
- Coordinate care received by clients, ensuring seamless transitions and effective management.
- Provide direct client care, adhering to established protocols and procedures.
- Implement phases of the nursing process, including assessment, planning, implementation, and evaluation.
- Document all information related to client care, maintaining accurate records and reports.
- Maintain open communication with healthcare professionals, residents, and families to address concerns and needs.
- Act as a liaison between clients, families, and physicians, facilitating effective collaboration and coordination.
Requirements:
- A valid LPN license in the state of New York.
- A minimum of 1 year of experience in long-term care or healthcare settings.
- Strong communication, interpersonal, and problem-solving skills.
- Ability to work independently and efficiently in a fast-paced environment.
- Proficient in electronic medical records systems.
