**Job Description:** Special Needs Care Coordinator
We are seeking a compassionate and organized Special Needs Care Coordinator to join our team at SUNY Upstate Medical University.
**About the Role:**
The Special Needs Care Coordinator will be responsible for coordinating care for patients with special needs. This includes engaging in daily meetings with case managers, communicating plans to patients and their families, and coordinating care with community providers.
**Responsibilities:**
- Coordinate care for patients with special needs
- Communicate plans to patients and their families
- Engage in daily meetings with case managers to conceptualize and develop treatment plans
- Participate in the implementation of treatment plans
- Manage and graph data
- Analyze data and identify key variables affecting patient outcomes
- Assist with daily living tasks while ensuring patient privacy and dignity are respected
**Requirements:**
- Associate's degree (or higher) in Psychology, Education, or a related field
- One year of related direct patient care experience required
- Ability to physically manage patients required
- Experience with Windows, Microsoft Office, Excel, Outlook, and behavioral data collection software required
**Salary:** $60,000 - $70,000 per year
